Application method
Pursuant to Article 13 of the Law and Article 5 of the Communiqué on the Procedures and Principles of Application to the Data Controller ("Application Communiqué"), you can submit your requests within the scope of your rights listed in Article 11 of the Law on Protection of Personal Data No. 6698 ("Law") to DORTEK KAPI SANAYİ VE TİCARET ANONİM ŞİRKETİ ("Company"), which acts as a data controller with one of the methods described below. This application form has been prepared only in order to respond to your request more easily and quickly. Other applications made provided that they comply with the procedures and principles set out in the Application Communiqué shall be evaluated and answered by the Company in accordance with the relevant legislation.

The Company has an obligation to determine whether the applicant data owner is the real right holder. In this context, the Company has the right to request all kinds of confirmatory information and documents in order to make identification. If your identity copy is forwarded to the Company in order to provide your identity confirmation, make sure that the sensitive personal data (blood group and religious knowledge) on the relevant document are not included.

We kindly request you to clearly write your request for your personal data within the scope of Article 11 of the Law below. Information and documents related with the subject should be attached.
Please make sure that the information and documents provided to the Company within the scope of your application do not include any sensitive personal data (e.g. health information, religious knowledge, political opinion knowledge) that is limited in number in Article 6 of the Law.
Finalization of the Application and Notification Method
Your application will be concluded free of charge as soon as possible and in any case within 30 (thirty) days at the latest, depending on the nature of the application from the date it reaches the Company. In case of an additional cost related to the finalization of your request, the information in the tariff determined by the Personal Data Protection Board
additional fees may be requested by you.
